Nomination Forms
Nomination forms will be available here starting on August 24, 2022 and closing on December 9, 2022 for members and closing January 13, 2023 for Section Boards.

All Nominees Must Meet Eligibility Requirements for an Award
Requirements are noted on the forms as well as HERE. Forms should be sent to the section president for their approval, and the section president should verify that the nominee meets the eligibility requirements for the award, including membership status. Membership status may be verified with CCAE at [email protected]
A list of past awardees can be found on the CCAE website (www.ccaestate.org) under the 2022 Awardees tab.
Complimentary Awards Meal
For those who receive an award at the State Conference, one free meal ticket will be provided for the awardee and one free ticket for a guest. If the recipient registers for the complete all-inclusive conference, his/her registration will be considered their personal ticket. Their one guest ticket will be in their conference registration packet. Any additional meals are the responsibility of the recipient. Sections may choose to help defray the expense of additional (i.e., family members) meals. Please notify the Awards Chairperson of any arrangements for extra tickets at least two weeks prior to the State Conference.
